What is a Patient Centered Medical Home, also known as, PCMH?

A Patient Centered Medical Home is a care delivery model whereby patient treatment is coordinated through their primary care physician. This is to ensure they receive the necessary care when and where they need it, and also in a manner they can understand.

North State Medical Center wants to be your medical home. Recently we have competed quality improvement (QI) projects in several clinical areas. Some of our improvement projects include: controlled blood pressure in hypertension patients, decreased hospital admissions, and increased patient satisfaction with follow up results.

In patients that were diagnosed with hypertension, there has been a 12% increase is controlled blood pressure. Hospital admissions were at an average of 10 patients per month in August-2016, in May-2017 we only had 1 admission. We realize that unnecessary hospital admissions lead to higher health costs and we want to help decrease those costs. While completing this QI project, we saw that there were a high number of unnecessary ER visits, and therefore our next QI project will be to decrease these numbers.

Our patients’ satisfaction is our utmost concern, and the best way to keep our customers satisfied is to listen to them. In October 2016, we gave out a patient survey in order to gather information to help us improve. We found that there was room for improvement with our follow-up on testing results. After working with our QI team and developing a plan for improvement we sent out the same survey in May 2017. We are proud to say we improved by 2%.
